Application for NDSU Pharmacy Student Intern Licensure
Application Instructions
- Complete all required information on the form. Make sure all information is complete and accurate to prevent any delays in processing.
- A pharmacy intern must license with the board of pharmacy when accepted into the Doctor of Pharmacy Professional Program at any board approved college/school of pharmacy and annually while successfully completing all four years of the Doctor of Pharmacy Program.
- Submit the form with $100.00 annual fee. This fee must be paid by credit/debit card.
- Once the application and fee have been reviewed, your registration will be approved and emailed to you at the email address that you provided on the application
